Why is Telehealth Important for Patients?
Telehealth equipment provides instant video access to a care coordinator who then connects with an RN case manager when appropriate. Telehealth monitoring is capable of checking weight, blood pressure, blood glucose, oxygen levels, and automatic medication reminder alerts ordered by a doctor or nurse.

By implementing telehealth within a patient’s care, we have been successful in reducing re-hospitalization rate by more than 15% while supporting patients in the best place to recover: at home or in a facility. The California average is over 18%, but patients with Wilshire’s telehealth access have an average rate of about 3%.
